ATTENDANCE & MAKE-UP POLICY
Attendance is necessary for progress in all classes. If for any reason a student is absent a phone call is greatly appreciated. Do not deduct for missed classes. If a child misses, he/she can make up the class at a later date. If your student is absent 4 consecutive times without notifying the office, they will be dropped from the program. Their space will be given to any new/waiting student. If the Raymore-Peculiar School District closes due to inclement weather, Studio M Dance Academy will also be closed. Missed classes may be made up within four weeks of the missed class and may not be accumulated beyond those four weeks. Classes may be made up in an alternate class of the same or lower level in which the student is enrolled. Please contact your instructor for a schedule of make up class times. Should you need to withdraw from any or all classes it must be done in person or in writing. A withdraw form must be filled out and signed by a parent or guardian. This must be completed 2 weeks before the start of a new month.

TUITION & REGISTRATION FEES
Tuition is due the first lesson for the month. A late fee of $10.00 will be added to all accounts not paid by the 10th of the month. There is a $25.00 charge for all returned checks. Students are enrolling in a full term, 15 weeks September - December and/or January - June, not individual classes. Tuition is the same each month regardless if it's a 3 or 5 week month. Payment for the term is divided into equal payments for your convenience. (Tuition is based on the total classes per term and then divided into monthly payments each term). Monthly tuition rates are not pro-rated due to holidays, inclement weather or missed classes; however, you are entitled and encouraged to schedule a make-up lesson for your child per the make-up policy. Registration fees must be paid at the time of enrollment. All tuition payments, costume charges, registration and recital fees are Non-refundable or Non-transferable.

DRESS CODE
All students are required to wear the proper dance wear and shoes in class at all times. Long hair must be properly secured back off the face. No jeans, big shirts or pants are permitted. No jewelry please. Students not dresses in proper attire may be asked to sit out of class. Please have all dance wear, bags and shoes labeled with student’s name. Shoes may be ordered through the studio for convenience. All orders must be paid in full when order is placed. Please see attire requirements by class listed below; Ballet – Leotard, tights, ballet shoes, hair pulled back off face. Ballet skirts and shorts are permitted. Jazz / Hip Hop /Pom Pon – Leotard or dance top, shorts or jazz pants. Jazz shoes or dance sneaker are allowed in class but not for performances. Hair pulled back off face. Tap – Clothing same as jazz. Jazz taps and hair pulled back off face. Combo Classes – Leotard or dance top, shorts or jazz pants. Jazz shoes, ballet shoes, and tap shoes (with elastic only, no ribbons permitted). Hair pulled back off face. Tumbling – Leotard and bare feet. Tight shorts are permitted. Clothes must fit snuggly for safety of students and teachers. Hair pulled back off face.

MISCELLANEOUS
Studio M Dance Academy reserves the right to reschedule or cancel a class time for reasons of insufficient enrollment or instructors’ professional discretion. The Studio is not responsible for personal injury of any student, or for stolen or lost articles. For Your Safety – Students must be picked up by a parent or guardian in our lobby immediately following class. Students are not permitted to wait outside. By signing below, I understand that there is a potential for injury with participation in any sport, including all classes at Studio M Dance Academy; and, while Studio M Dance Academy, its owners, directors and teachers, will make every reasonable effort to eliminate potential for injury, such injury may still occur. I understand this risk and agree to hold Studio M Dance Academy, its owners, directors, and teachers harmless from any and all liability connected with any injury arising out of participation in classes at Studio M Dance Academy.
